By entering the duct with the shortest and stiffest probe first, your doctor can penetrate strictures on the surface of the duct and within the first millimeter of the gland. These strictures are created by periductal fibroses, or scar tissue, around the gland, which squeeze the duct and alter the needed channel through which oil flows. Without the proper flow of oil to the eye, the gland becomes progressively blocked, and the patient can feel increasing inflammation, tenderness in the lid, and additional symptoms associated with MGD and dry eye.</image:caption>

The build-up of inexpressible meibum leads to elevated pressure behind the blockage, causing tenderness in the lid and spurring deterioration of acini and reduced oil production.
